Insurance

As a courtesy to you we will bill directly to your primary insurance company. We do not bill secondary insurance except for Medicare patients.

We also contact your insurance company to verify benefits. This informs you of any limits, special requirements, etc.

However, this is not a guarantee of coverage as we are only able to communicate what your insurance company has told us and you are ultimately responsible for any amount that your insurance does not pay.

Billing

If you have a copay we ask that you make a payment before each visit. We accept cash, personal checks, Visa, Mastercard, and Discover. Once your insurance company processes your claim, we will bill you for any remaining balances.


Medicare

---

If you have Medicare as your primary insurance, we will bill a secondary insurance for you. Please feel free to call our Insurance Verification Department (Ext. 21) for further explanation or if you have any questions.


Worker's Compensation

---

For our workers compensation patients we cannot make appointments until we have received approval. Please call ahead and be sure to have your: date of injury, claim number, adjusters name, adjusters phone number, adjusters fax number, nurse/case manager name, and attorney information (if applicable). Please also be aware that our insurance staff works closely with your nurse/case manager, and any no-shows and cancellations will be reported to them.
