For our workers compensation patients we cannot make appointments until we have received approval. Please call ahead and be sure to have your: date of injury, claim number, adjusters name, adjusters phone number, adjusters fax number, nurse/case manager name, and attorney information (if applicable). Please also be aware that our insurance staff works closely with your nurse/case manager, and any no-shows and cancellations will be reported to them. Please feel free to call our Insurance Verification Department (Ext. 21) if you have any questions.
